Did contact person make a quick review of the Primary Funeral Information?

Did family collect the following information to proceed?

1. Name, phone number, and email address of contact person to share.
2. Name, age/sex, day & date, and place of death of deceased.
3. Location of deceased body – hospital, hospice, nursing home, or home?
4. Has death or transportation certificate been obtained from a doctor, nurse etc. to be used by the funeral home? If not, make sure it is done.
5. Desired burial date by the family/friends?
6. Funeral Home contact information from www.alim.org or mosque.
7. Cemetery contact information from www.alim.org or mosque.

Contact funeral home to pickup the body from the place of demise. You may be signing their contract with payment prior to washing and preparation of the body.

Contact cemetery to provide timely burial services on the date and time family desires. You may be signing their contract with payment prior to actual burial.
